If you request disciplinary information for your own use, please use our ONLINE SYSTEM​ to make your public information request and payment.

Fees for Public Information Requests

The Board charges a $20.00 fee for record retrieval and the first 50 pages. There is an additional charge of $.50 per page for each page over the first 50.
